Alternatives to an IVC Filter
Patients with medical histories of deep vein thrombosis (DVT), venous thromboembolism (VTE), and pulmonary embolus (PE) are evaluated for the potential of a variety of treatment options. Typically these include medicinal programs, extremity compression treatments, retrievable inferior vena cava (IVC) filters, and permanent vena cava filters.
These underlying conditions are significant causes of injury and mortality in relatively immobilized, hospitalized patients, and have not insignificant rates of occurrence in the general population. The inferior vena cava leads blood from the lower limbs to the heart and lungs where it is refilled with oxygen and then sent through the circulatory system again. In patients with high coagulation level risk factors, complications arise when blood clots move from lower limbs to the heart and lungs (i.e. PE). For these health risks physicians make assessments for treatment, and usually begin with alternatives to implanting IVC filters.

Treatment Options
Anticoagulation therapy is usually the first line of treatment for patients with those medical histories and conditions that make them prone to the conditions listed previously. Patients who are immobile for extended periods of time are a prime population for consideration for this type of treatment. However, not all patients tolerate this type of treatment, and not all patients who are treated with anticoagulants such as Warfarin respond well enough to this option. Medications can be administered orally or intravenously.

Compression treatment is also a standard treatment in patients at elevated risks for blood clots and the associated PE. Hospital settings routinely use this type of therapy, and patients can also easily apply compression stockings in the home setting.
In patients who do not find sufficient protections against the threats that blood clots pose to the pulmonary system, alternatives to these more conservatives include retrievable and permanent IVC filters.

Retrievable IVC filters are the newer of these options. In this procedure the IVC filter is inserted into the inferior vena cava below the renal system. Here it is designed to collect blood clots and prevent PE. After a relatively short period of risk time, such as when a patient has a determined large clot in a leg which is only mildly responding to anticoagulants at the time, the filter is retrieved after the danger has passed. This is done through a catheterization process and is minimally invasive.

In other patients who have long-term struggles with blood clot formation and their inherent risks, permanent IVC filters may be the best option. Through a similar process the filter is inserted via catheter. The filter is deployed and is expected to prevent clots from passing through the inferior vena cava. Only when complications arise, such as a fracture in the filter or in thrombosis at the site are permanent filters removed.
