Inferior Vena Cava Filter: An Approach to Thrombosis
To prevent the migration of thromboses – embolization – an inferior vena cava (IVC) filter is used in some patients. The IVC brings blood from the extremities to the heart and lungs. This filter prevents the embolized clots from entering the lungs and causing a pulmonary emboli. Using guided X-ray or ultrasound technology, the filter is deployed in the large vein, the IVC, within the abdomen.

The patients: Most of the time the filter is used to treat patients who have thromboses in the legs, known as DVT, or deep vein thrombosis, and those patients with a history of pulmonary embolism. Even those patients who have never had these occur might be considered for the procedure if they are at increased risk of a PE. Patients who struggle with clotting factors and cannot effectively take anticoagulants are also sometimes clinically eligible for the device.

Tools for the Procedure: Common IVC filters are constructed rom stainless steel or a variety of titanium compounds. There are two general categories of filters: those that are designed to be retrieved at a later date, and those that are designed to be permanent. A catheter is used for the insertion. Contrast dye and imaging technology are used in tandem. Before the procedure an EKG, abdominal CT, ultrasound of the legs, or other tools might be used.

The risks: As with any type of procedure where there is a certain level of invasiveness, there is risk of infection and complications such as excessive bleeding or venous perforations during the filter placement. Although it is rare, there are instances of migrating filters, fracturing filters, and filters which become blocked with heavy thrombosis, restricting blood supply.

The procedure: Usually the groin or neck (jugular) area is used. The site is cleaned and numbed, and the patient is either lightly or completely sedated. A catheter is inserted. Contrast solution is administered and guided by imaging. The filter is sent to the abdominal area and deployed, or opened. It should be able to work immediately. The catheter is removed. Pressure is applied to the site for five to ten minutes. A small bandage is applied and the patient is sent to recovery for an hour or two. Then the patient is either discharged or sent to an overnight patient room.

The recovery: Patients need a ride home from the facility. They can usually resume normal eating and activity levels within a day or two, but should refrain from heavy lifting or strenuous exercising for the first three days, unless otherwise instructed. Beyond this, there should be no extra precautions beyond looking for signs of complications. These include: swelling, bleeding, a fever, chest pain, and difficulty breathing.
