What Are Inferior Vena Cava (IVC) Filters?
Inferior vena cava filters are medical devices implanted within the inferior vena cava, the largest vein in the human body. This device is used in patients who are at increased risks for pulmonary embolisms or who suffer from large clots in the inferior vena cava. The design of the filter is such that it is supposed to collect the blood clot before it can reach the heart or lungs.


The Inferior Vena Cava
As the largest vein in the human body, the inferior vena cava (IVC) is responsible for delivery de-oxygenated blood to the right atrium and then on the lungs. This de-oxygenated blood is collected from the lower extremities, which is why clots in locations such as the legs pose risks to the vital organs of the heart and lungs. If one or both of the pulmonary arteries is affected by a blood clot it is known as a pulmonary embolism.
Pulmonary embolisms pose significant threats to patients. The blockages can interfere with breathing, result in chest pain, and even cause death. Some patients have conditions that predispose them to forming blockages. Other patients are unable to use medications that reduce this risk because of health conditions that are not compatible with thinner blood and increased risks of bleeding.


The IVC Filter
In order to stop blood clots from passing to the heart or lungs, the inferior vena cava filter is designed as a wire basket or filter, inserted into the inferior vena cava during a surgical procedure. This procedure involves making a small incision into the vein either through the neck or groin area. Often this surgical procedure is done as a same-day, outpatient procedure.


A local anesthetic is used to numb the area of the insertion, but a majority of patients remain awake during the procedure, only lightly sedated or given medication for relaxation. During the procedure the surgeon threads a very thin guide wire while a contrast dye is used in conjunction with X-rays to guide the surgeon. Once the IVC filter is in place, the surgeons withdraws the insertion catheter. Patients usually are able to resume normal activity within 24-48 hours of the procedure, depending upon other health concerns.
Inferior vena cava filters are designed to remain within the human body unless there is an unforeseen complication. Some of these complications might include the IVC getting lodged in the wall of the inferior vena cava, or if the basket of the device becomes too filled with large blood clots. The longer a patient has the IVC filter in place, the more challenging the removal of the device will be.
