FAQ

IVC Filters: Questions for Physicians
Populations of patients proposed as candidates for inferior vena cava (IVC) filter placement need to fully assess their medical situations. This involves communicating thoroughly with physicians. Physicians should be prepared to provide detailed information to patients. Not all patients will be prepared to formulate the kinds of questions necessary for engaging in proactive discussions about healthcare management, but the following should be considered minimal information necessary for an informed decision.

How does the inferior vena cava (IVC) filter help manage thrombosis? Thrombosis is a condition where blood clots develop within the circulatory system. The filter is designed to prevent moving blood clots – or embolized clots – from entering the delicate systems of the lungs or heart where the clots can cause significant damage.

What is the design of an inferior vena cava filter? Made from a strong metal such as stainless steel, the IVC filter has teeth like a comb but is shaped like a cone. When it is inserted the filter is closed, but when the technician locates the place in the IVC in the abdomen, the filter is opened.

Who receives the IVC filter? The filter is intended for use in patients with complications from thrombosis. A condition known as deep vein thrombosis puts patients at increased risks for pulmonary embolism, where a clot that develops deep in the veins of the leg, for instance, travels to the lungs. Patients often first receive medication to control thrombosis. However, not all patients are medically eligible for this course of treatment. Patients who are hospitalized for an extended period of time are susceptible to thrombosis as well, and an IVC filter may be the best course of treatment.

What should patients expect from the procedure to implant the IVC filter? The procedure to implant the IVC filter in the inferior vena cava in the abdominal area uses a vascular technique. Through a major vein in the leg or neck, a technician inserts a catheter. Through the catheter the technician or vascular surgeon sends the filter. Ultrasound or X-ray is used to help find the best location. Once there, the filter is opened and it is essentially ready to start working immediately. The surgeon removes the catheter.

What is involved in recovery from the IVC filter surgery? Recovery from the IVC filter surgery is generally minimal and is free of such things as rehabilitation. There are no sutures required. Sometimes skin glue is used. Patients will receive general skin cleaning instructions for the catheter insertion point. Patients may notice bruising or slight tenderness at the site. Recurrent or moderate to severe bleeding is a complication and patients need to report this immediately. Patients should not do any heavy lifting after the procedure for at least three days. After that usual activity is often tolerated.
