FAQ

IVC Filters and Aortic Perforations

IVC filters are used to manage deep vein thrombosis and the risk for blood clots breaking free and traveling to the lungs, causing a pulmonary embolism. Permanent filter placement has been used for decades, but recently doctors have begun prescribing retrievable filters, intended to treat short-term risk for pulmonary embolism. Unfortunately, these retrievable filters, especially when left in place longer than the recommended amount of time, put patients at risk for a variety of health complications.

One of the complications that can occur when a filter breaks free from its original placement and moves through the body is aortic perforation. Aortic perforations are tears in the inner and out walls of the aorta. These can lead to sever bleeding and because the aorta is near the heart, pressure can affect blood pumping and cause a rupture. Aortic ruptures are extremely dangerous and potentially fatal.

Dealing with IVC Filter Risk

So what should you do if your doctor has recommended an IVC filter to help you deal with the risk for pulmonary embolism? First, find out if you are treating a temporary or permanent risk. Permanent IVC filters can create problems, but pose much less risk than retrievable filters.

If you are considered an ideal candidate by your doctor for a retrievable IVC filter, get a second opinion if possible. This is not always an option because time is of the essence when treating the risk for pulmonary embolism, but if you are able to seek another opinion, do so.

Retrieving the IVC Filter

Also be sure to discuss the potential risks when using an IVC filter. Many of these risks are related to leaving the device in place for longer than recommended. If your doctor wants to place a retrievable IVC filter to help you reduce your risk for pulmonary embolism, ask when the device can be removed. Ideally, IVC filters are removed the moment the risk for pulmonary embolism has passed.

In addition to aortic perforations, there are a number of other health risks that are of concern when using an IVC filter. You want to discuss all of these with your doctor in advance because many of them are potentially fatal. Many can also be avoided when IVC filters are used properly. If a filter is your only option because you are unable to take anticoagulant medication, you need to be sure you prepare properly. Knowing your risks and knowing the warning signs of a problem are essential.
