What Role Do IVC Filters Play in Treating Pulmonary Embolism?
IVC filters are inserted into the inferior vena cava with the intention of preventing blood clots from traveling to the lungs and into other parts of the body where they can cause serious medical complications. The filters are viewed as a last resort by many doctors because they do feature a variety of risks.


Ideally, patients at risk for blood clots and pulmonary embolism are treated with anticoagulant medication. This is medicine that thins the blood, so there is very little risk of it clotting. Unfortunately, not everyone can use anticoagulants. Some people are dealing with medical situations or diseases that are contraindications for anticoagulants and others have tried the medication and experienced few results. When this occurs, IVC filters can be used to provide protection against the risks associated with blood clots.


How IVC Filters Block Embolisms


IVC filters work by blocking clots in the vascular system. It should be noted, with an IVC filter, clots can still form just as easily as they would without the filter (some argue filters even increase the risk for pooling or clotting blood). Whereas anticoagulants prevent clots from ever forming, IVC filters do nothing to stop their formation, but do block their movement through the body. The filters hold the clot in place until the body is able to dissolve it naturally. Filters also allow for normal blood flow around the clot, so if functioning properly, a person can lead a normal life without concern for pulmonary embolism.


Unfortunately, these filters do not always perform as expected. In addition to not blocking 100% of blood clots, filters can also dislodge and travel to other locations in the body. It is also possible these filters will break off and the broken part moving throughout the vascular system can puncture or perforate the tissue.


Retrievable and Permanent IVC Filters
There are two types of IVC filters: permanent and retrievable. Retrievable filters are placed in patients who have only a temporary risk for pulmonary embolism. For instance, the risk for blood clotting increases after injuries and surgery, so a filter might be placed until that risk passes. Despite these devices being marketed as retrievable, many of them can be difficult to remove. In some cases, patients and their doctors just don’t bother removing the filter unless it causes a problem. The complications from leaving retrievable filters in place has grown so severe the FDA now recommends against it.
