What is IVC Filter Perforation?
There are several complications that can occur with the use of an IVC filter and one of the most common is filter perforation. Some studies have shown that as many as one quarter of all IVC filter users suffer a perforation incident either in the vena cava or in an organ. Perforations can occur and not be serious, but there are also times in which a perforation is very serious, such as when it affects the intestines, aorta, or lungs. These perforations can be life-threatening and at the very least, require surgery to repair.
IVC filter use has increased a great deal in the last 20 years, due in part to the development of the retrievable filter. Retrievable filters are meant to be used on a temporary basis, when a person faces a heightened risk for pulmonary embolism that is not expected to last more than a few weeks or months.
How Does Perforation Occur?
Filters come in a variety of shapes, but all of them feature legs or struts made of metal wire. These pieces are known to erode or fracture, eventually causing perforation. The most common perforations occur within the vena cava, but there are also a number of reported perforations in the duodenum and the aorta.
The problem is so severe the FDA issued a warning in 2010 concerning retrievable filters, which are considered more likely to fracture and cause perforation. The warning stated that anyone with a retrievable filter should have the filter removed in the appropriate amount of time. The FDA has received approximately 1000 adverse events reports, including nearly 100 related to perforation.
Symptoms of Perforation
Sometimes there are very few symptoms associated with perforation. Patients might not even realize there is a problem. Other times the perforation causes major complications and the perforation is immediately known. Common symptoms associated with perforation include pain, bleeding, swelling, cramping, rapid heartbeat, and shortness of breath. The specific symptoms will depend on where the perforation occurred and how it affects your body’s internal functions, if at all.
If you suspect you have an IVC filter perforation or you believe anything else is wrong with your filter, you need to contact your doctor as soon as possible. Some of the medical complications of filter perforation require immediate medical attention, so you should go straight to the emergency room. It is important to never ignore symptoms when you have an IVC filter because they might be signs of something very serious.
