IVC Filter Side Effects

Why are IVC filters removed?
Filters placed within the inferior vena cava (IVC) to aid in the reduction of risks associated with thrombosis and embolization of clots are either permanent in nature or removable filters. Some removable filters, in fact, end up needing to be left in a patient permanently. There are several indicating factors which help physicians determine whether or not an IVC filter should be retrieved or left as a permanent proactive treatment option.

Reasons for IVC Filter Removal
The main reason why a patient demonstrates indications for filter removal is that the risk of embolization within the pulmonary region has lowered to a significantly safer level. The reasons why this occurs can be of the following in nature:

- The patient begins to respond well to anticoagulation therapy, whereas he or she did not have a high enough response rate initially. Common medications patients will use include heparin and warfarin.
- The patient’s specific mobilization circumstances change. This could occur, for example, in patients who first received the filter after an injury, during pregnancy, or for another reason of short-term immobility or chemical changes within the body.
Although these instances occur on a much more rare scale, there are circumstances where the IVC filter needs to be removed because of a health risk to the patient. Examples of this include:
- A filter that is it not placed in the correct area during the initial procedure. An interventional radiologist might discover this finding on the first follow-up set of imaging and the patient must then have a removal procedure done to correct the issue.
- A filter that breaks within the inferior vena cava. This is a rare event, but it still poses health risks to the patient. Retrieval of all parts of the filter must be done as quickly as possible.
- A filter that migrates from its original and proper location. This is also a rare event, but it can pose serious health threats, including death, as a free moving filter or filter piece can end up all of the way in the pulmonary system.

Whenever a physician determines that it is time to remove a filter, it is essential that the patient demonstrate at least several weeks of risk free circulation. This is because there is clinical evidence to suggest that pulmonary embolisms can occur up to a month after a significant health event.

It should be noted, however, that some patients who do medically qualify to have the filter removed cannot tolerate the procedure because the positioning of the filter is such that removing it can do more harm than good. As the vena cava adapts to the metallic structure, it might so securely attach to it that removing it is a risk of hemorrhage.
