Are the Risks for Permanent and Temporary IVC Filters the Same?
For years, the only option patients at risk for pulmonary embolism had for using an IVC filter was to have the device implanted permanently. More recently, medical device manufacturers have developed a temporary option for treating pulmonary embolism risk with a retrievable IVC filter. These filters are implanted the same way as permanent filters, but are intended for short-term use and should be removed once the risk for pulmonary embolism has passed.


Risks of Retrievable Filters
It might seem as though the risks would be similar for these devices would be the same, since in many ways the devices are the same. Surprisingly, this is not the case at all. The risks for retrievable IVC filters is far greater than it is for permanent filters.
Retrievable filters are designed to be temporary, but some manufacturers have stated they can be used liked permanent filters. This was the response to incidents where temporary filters proved difficult to remove because of scar tissue. The FDA strongly urges the removal of temporary filters as soon as the risk for pulmonary embolism has passed. This avoids the risk of scar tissue forming and reduces the changes other serious side effects will occur.


Migration and Fracture
In addition to the complications related to implanting the IVC filter that are present for both temporary and permanent filters, there are additional risks with the retrievable filters. Two of the most common include the device or parts of the device moving to other locations in the body. Device migration occurs when the IVC filter detaches or was never attached properly to the vena cava wall and migrates to another part of the body. The risk is it could puncture soft tissue or an organ, or lodge in a location where it blocks blood flow.


It is also possible for the IVC filter to fracture. When this occurs, the broken piece can migrate and create some of the same problems that occur when the entire device moves.
Once the device fails, patients are once again left at risk for pulmonary embolism – just as much, if not more, than they were before implantation.  If you are considering the use of an IVC filter to manage the complications related to blood clots or your doctor has suggested an IVC filter might be necessary, make sure you know whether it is a permanent filter that will be used and what risks you need to be concerned about once the filter is in place.
