7 Reasons Why an IVC Filter Should Not be Removed
While there are inferior vena cava filters designed for retrieval after risk factor subside, there are also indications for the continued use of filters in patients. These indications can exists regardless of the type of filter originally implanted in the patient.
Inferior vena cava (IVC) filters are designed to hold clots within the vena cava – the largest vein in the body – so that these clots do not reach the lungs and cause a pulmonary embolism (PE). A pulmonary embolism is a life-threatening medical condition. IVC filters give patients protection from embolization of clots, but not from the direct development of such clots.

The reasons for determining that patients could medically benefit from filters within the vena cava are varied, but there exist some common reasons why patients should not have these filters removed.

1. The patient continues to produce blood clots. There are many reasons why this happens. Some patients have family histories that strongly suggest continued struggles with thrombosis. Other patients continue to be immobile and therefore continue to be at risk for clot development.

2. The patient is still unable to effectively use anticoagulants. Anticoagulant treatments such as Coumadin offer a first line of protection against clots. In patients who cannot tolerate this type of pharmaceutical treatment, continued filter use is often indicated.

3. There is a likelihood of filter need in the future. Patient needs for filters can ebb and flow. During some points in their medical picture the instances of thrombosis are reduced, while other times, such as for surgical procedures, the instances increase. Patients who have predicted medical situations that make clotting more likely should not have their filters removed prior to this event.

4. The patient is struggling to maintain a medication schedule and healthy lifestyle. Patients who react well to anticoagulants but who do not follow the prescriptions for healthy choices are at increased risks for clot development in the future.

5. Tests reveal that the filter is significantly imbedded in the vena cava. This can make removal more dangerous and risks include perforation of the vena cava and serious bleeding.

6. Tests reveal that clots still exist in the patient. Even if obvious causes for thrombosis are not found, the presence of any clots warrants leaving the filter in place. Tests such as ultrasounds can help physicians determine this risk levels.

7. Patients are not expected to have a long life expectancy. In patients who show signs of nearing the end of life, either due to age or health or a combination of, should be considered for filter continuation as to not pose more risks to the already ailing body.
