Vena Cava Filters: An Overview
The use of a metal filter to protect a patient against the potential complications associated with embolization of blood clots is one treatment option for deep vein thrombosis (DVT), and other conditions which result in clot formations.
The vena cava filter structure: Filters are made from metal. They can be either used temporarily or permanently, depending upon the condition of the patient. The filter design includes small barbs that help to stabilize the filter within the vena cava. The filter shape is available as one that resembles a let V or one that resembles a nest of a bird.
Indications for insertion of an IVC filter: Conditions which indicate the insertion of an IVC filter are multiple. For some, the use of medications such as Coumadin is not a viable option. There are certain other health conditions which prevent anticoagulants from being safe options. Examples of these are pregnancy and injury which cannot tolerate increased levels of bleeding.
Equipment utilized in IVC filter placement: A catheter, which is a narrow tube-like device, is used by physicians to send the filter into position. An X-ray or ultrasound machine is used with contrast solution to provide imaging for the placement. A laser tool can be used during the retrieval process.
The IVC filter healthcare team: A team of healthcare providers work together during the IVC filter placement. Vascular surgeons usually lead the process. They are assisted by nurses who provided direct care and monitoring, and radiologists who assist with the imaging. An interventional radiologist often reads the results of the image studies to make sure that the IVC filter is in place.
Placement of the IVC filter: The placement procedure involves accessing the vena cava via a major vein in either the neck (jugular) or leg. The catheter is launched in the vein and sent to the vena cava. The radiologist ensures it is in the precise location, near the kidneys. The filter opens up and essentially grabs the interior lining of the vein.
Removal of the IVC filter: In instances where removal is required, a similar procedure takes place. If the filter has been in preventative use for an extended length of time, a laser procedure might be needed to more safely remove the device.
Recovery: A patient should not expect a lengthy recovery from the placement procedure. Only limited post-op care is needed. Most patients return to daily activities within 48 hours, but there may be limitations placed on lifting objects.
Risks: Risks of IVC filter use include damage occurring to the vein accessed during the procedure and malposition of the device. Risks also extend into recovery, including a separation of the filter components. These parts, or the filter in its entirety, can move dangerously within the inferior vena cava. Immediate intervention is needed.
