IVC Filters and Fractures
IVC filters are used to reduce a patient’s risk for pulmonary embolism, but warnings from the FDA could change the way doctors prescribe filters to patients. After receiving hundreds of reports concerning adverse events related to retrievable IVC filters, the FDA is now concerned doctors are not removing filters as they should and patients are suffering unnecessarily.
As a result of the problems, the FDA is now recommending all retrievable filters be removed as soon as the risk for pulmonary embolism has subsided. This reduces the likelihood the device will fracture or migrate, both of which seem to happen more frequently the longer a device is left in a patient.
Study Shows Bard IVC Filters Especially Risky
One study that included 80 IVC filter patient at a Pennsylvania hospital showed there was a particularly high risk among recipients of the Bard IVC filter. The study showed about a quarter of the patients with the Bard Recovery system suffered fragmentation and embolization of their device, as well as about 12% of those with the Bard G2 device. Researchers determined that about 7000 recipients of the G2 filter are at risk for embolization.
Researchers believe the incidence is less in the G2 IVC filter because Bard made modifications to reduce the ability of the parts to fragment and embolize. Unfortunately, it is not enough to stop fracturing completely and patients are still at risk.
Preventing Complications from IVC Filters
There is no way to completely eliminate the risks that are present when an IVC filter is placed. For some who suffer a high risk of pulmonary embolism, the potential for problems from the filter is worth the risk to prevent the potentially deadly condition.
However, the FDA has recommended doctors take all necessary precautions to prevent problems by using the devices properly. In addition to removing IVC filters within the recommended period of time, it is also important doctors only place the devices when patients fit the criteria for IVC filters. For instance, they should have a high risk of pulmonary embolism and be unable to use anticoagulant medication for one reason or another. There are cases in which patients were given IVC filters unnecessarily and now they are suffering because of it.
If your doctor has recommended an IVC filter, be sure to ask if there are any alternatives for treating your pulmonary embolism risk. Also ask if there is anything that can be done to reduce the risk of device failure if it is your only choice.
