Preparing for IVC Filter Removal
Physicians who determine that the benefits provided patients with inferior vena cava (IVC) filter no longer provide enough evidence for use may recommend the removal of the filter. Before this is done, however, assessments and tests should be performed to ensure that the best decision is made for each patient.
Assessing Filter Benefits
Physicians should first revisit the reasons why the patient first needed the filter. This original diagnosis needs to be reassessed. Some of these reassessments are quite cut and dry – that is there are obvious indications that the original risks for pulmonary embolisms have been mitigated. Patient profiles in these situations could be pregnant women who have given birth and individuals who have fully recovered from injuries that required hospitalization and immobility.
It is important to note, however, that on occasion the initial reason for the filter might have resolved, but a new risk factor has emerged. It could also be that a patient is not immediately facing risk factors, but knows that in 3 months he will need back surgery and his risk for thrombosis will once again rise.
Preparing for the Removal
Once the assessment is concluded and the physician feels that the patient fits the criteria for effective and safe removal of the filter, tests should be performed to fully confirm those decreased risk factors. Examples of these tests include:
- Blood tests to monitor coagulation. If anticoagulant medications are being used, these should have a consistent track record for the individual patient of effectively managing clotting factors.
- Duplex ultrasounds to measure blood flow. These tests can help physicians review the structure of the patient’s vein and make sure that clot formation risks do not exist.
- Venograms to review the anatomy of the veins. These imaging studies can also sometimes indicate whether or not clots still exist deep within the veins (DVT or deep vein thrombosis).
- Imaging studies of the abdomen. This is done to verify the location of the filter within the inferior vena cava. The vascular surgeon or interventional radiologist should not go into the removal procedure without first identifying the current location of the filter.
Beyond these pre-operative tests, patients will also need to follow instructions similar to those given for the placement surgery. These include restrictions on diet to prepare for sedation, clear medication histories, and arranging a ride to transport him or her after the procedure. Patients will again lie on their backs, have a catheter placed in a main vein, and a technologist send a device through the catheter.
Recovery for the removal procedure is similar to that of the placement operation, with a couple of days of rest and limited lifting. A more strict recovery plan might be warranted if there was significant bleeding during the filter removal.
