IVC Filter Side Effects

Has the FDA Issued an IVC Filter Warning?
IVC filters are used to manage deep vein thrombosis. This is a condition in which blood clots develop in the deep veins of the body, usually in the arms or legs. When clots develop in the legs, they can break free and travel through the body to other locations. The biggest concern is if they reach the lungs they can lodge in place and create a blockage, preventing blood flow from the heart to the lungs, which can be potentially fatal.

IVC filters work by capturing the blood clot that breaks free in the vein and prevent it from reaching the lung. It is held in place until it dissolves and no longer poses any sort of threat. IVC filters are intended for use when no other treatment is working. It is possible to take anticoagulant medications that prevent blood clots from forming, but when these do not work or they are not appropriate for a particular situation, IVC filters offer some protection against pulmonary embolism.

Though IVC filters are supposed to be used as a last resort, they are actually quite popular and continue to increase in popularity. Part of this is due to the creation of retrievable IVC filters. These filters work in the same way as permanent filters, but they are intended to treat only the short-term risk for pulmonary embolism.

Trouble with Retrievable Filters
Retrievable filters can be effective, but they can also be dangerous, especially if they are left in place longer than intended. Most retrievable IVC filters should only be used for a few weeks at most, but there are instances in which doctors have not removed the filters, even though they were only a temporary treatment.

FDA Gets Involved
The problem was so prevalent the FDA was receiving reports about it. There are approximately 1000 adverse event reports on file with the FDA concerning IVC filters, most of which relate to retrievable filters and many of which were caused by leaving the filter in place too long.

As a result of all the reports, the FDA eventually released an official recommendation concerning retrievable filters. It encouraged doctors to remove the filters as soon as possible, once the risk for pulmonary embolism had passed. The point was to encourage urgency of removal, even if the device manufacturers stated or implied retrievable devices could be treated like permanent ones. The FDA later updated the recommendation to include specific time frames for removing IVC filters.
